It´s the normal way for me, I plan it in my mind and when I finally sits down and type it there will just be smaller adjustment. I like the idea of notes and I always have a notebook with me. They typically become rather torn after a while but yesterday I found what I have been looking for some time. A leather wrapcover with place for notebook and pen. Found it on Etsy, a women i UK was selling her handcrafted cover so it was an easy buy.
